The story behind Purnima Vrat
Purnima, the full-moon tithi that closes the bright fortnight, is observed every month with a fast and, in many homes, the Satyanarayan Katha. The full moon is held to be the time when the mind is most receptive, so charity, japa and a night vigil under the moon are all counted as especially fruitful. Several great festivals — Guru Purnima, Raksha Bandhan, Sharad Purnima, Kartik Purnima — are simply this vrat carrying a special name.
Purnima Vrat Puja Vidhi

Morning Sankalpa
Bathe before sunrise, ideally in a river or with Ganga jal added to the water, and take the vow to fast until the moon is sighted.

Vishnu or Satyanarayan Puja
Worship Lord Vishnu with tulsi, yellow flowers and panchamrit; families often read the Satyanarayan Katha in the evening.

Chandra Arghya
After moonrise, offer arghya of water and milk to the moon and chant the Chandra mantra.

Daan & Parana
Give food or cloth in charity, then break the fast with a satvik meal after the moon offering.

Salutations to Lord Vasudeva, the all-pervading one.
How to chant
Chant 108 times during the puja; on Satyanarayan Purnima recite it before each chapter of the Katha.
